In more ways than one, the existing healthcare system leaves its users in a tough spot. Recognizing these gaps means making healthcare all the more satisfying and accessible. One of the highly noted areas in this aspect is kidney care. Contemporary kidney care focuses almost entirely on ESKD and in-center dialysis, missing the primary drivers of high cost and poor outcomes.
The focus of Strive Health is to change the kidney care paradigm to identify patients earlier, prioritize the right care at the right time, and drive better outcomes. Most importantly, all of this is done while lowering costs. Thus, it is no surprise that Strive Health is the nation’s leader in value-based kidney care and partner of choice for innovative healthcare payors and providers seeking to improve the lives of their kidney patients.
With a mission to deliver compassionate kidney care the way it should be done, Strive Health emphasizes data-driven insights, high-touch care, prevention, and next-generation dialysis services. For many healthcare companies, the ambition to achieve their goals overtakes the core of their purpose. However, Strive Health takes extensive care to keep to its guiding values such as tenacity, excellence, and innovation, while serving patients, partners, and the broader community.
Since its inception five years ago, Strive Health has taken every available opportunity to deliver compassionate kidney care the way it should be done. Having stayed close to their mission, Strive Health was able to reach yet another milestone in 2023. Last year, Strive Health had the opportunity to launch a kidney care collaboration with Trinity Health through Loyola Physician partners. As one of the nation’s largest not-for-profit Catholic health systems, Trinity Health and its member Loyola Medicine involves a nationally ranked academic, quaternary care system.

Apart from this, Strive Health continues to make kidney care more simple and better for the world. As of now, the company manages over 56,000 patients with CKD and ESKD through partnerships with nephrologists and other care arrangements with payors and providers. Earlier in 2022, Strive Health signed an innovative nephrologist-led partnership with the nation’s largest nephrology group, Nephrology Associates of Northern Illinois and Indiana (NANI).
When the patient’s options have been typically limited to dialysis or a transplant for decades, innovating the field is more than crucial. Using predictive analytics and machine learning, Strive Health’s technology gathers data from a variety of sources to gain a holistic view of the patient’s experience.
Armed with this information, Strive’s Kidney Heroes™, which is an interdisciplinary clinical care team including nurse practitioners, dietitians, pharmacists, care coordinators, and licensed clinical social workers uses that information to gain a better understanding of the patient’s risk of hospitalization or the progression of their disease.
These team members act as an extension of the patient’s physician to help manage both kidney disease and comorbid conditions, such as diabetes. The lack of such streamlined patient care more than often impacts a patient’s overall health. With Strive Health, the result is less frequent hospitalizations, fewer readmissions, lower costs, and better outcomes. Innovation by Strive Health also gives way to enhanced patient experience, positioning healthcare providers to succeed in the future of kidney care.
